You can change your PCP if you need to. To change PCPs, call your Health Plan .
You will be asked to choose a Plan when you apply for Medicaid or LaCHIP. The Health Plan that you choose will also provide any behavioral health services if you or your family member should need them. You will be enrolled into the Health Plan you've chosen. If you did not choose a Health Plan, we will choose one for you. Bayou Health will send you a confirmation letter in the mail telling you which Health Plan you've been enrolled in. You can change Health Plans if you no longer wish to stay in the plan you've been enrolled in.
To change Plans during the first 90 days:
- You can change Plans on line using the Choose or Change Here page on this site.
- Call Bayou Health at 1-855-BAYOU-4U (1-855-229-6848) and say you want to change Health Plans. Some of our agents speak Spanish, and we have free interpreter services for other languages.
After the first 90 days: You can change Plans for certain reasons, such as, if your Plan changes services that you need, or if you cannot get certain care that you need. Call Bayou Health and ask for a transfer form. You cannot change Plans online if you are changing for such reasons.
Once a year: You will have a 60-day time period in which you can change Plans if you want to. This time period is called open enrollment.
If you have a complaint about your Health Plan or PCP, call your Plan's customer service department.
