How to Enroll

Enrolling means to sign up and make your health care choices. You must be in Medicaid or LaCHIP to enroll.

When to enroll

Bayou Health is the way most of Louisiana's Medicaid and LaCHIP recipients receive health care services. We want to handle your enrollments with care.

Bayou Health will send you a packet of information in the mail telling you when to join a Health Plan. After you get your packet you will have about 30 days to enroll.

To enroll, you will need to tell us your Social Security number and your birth date.

Enroll at Medicaid Eligibility Office or a Medicaid Application Center

3. Enroll on paper

Fill out the enrollment form that comes in your packet from Bayou Health. Either mail it back in the postage-paid envelope from your packet, or FAX it to
